I have worked in a healthcare role since I was last registered
You'll need to send us a statement, known as healthcare services statement, from your employer(s).
You can download a copy of the statement for your employer to fill in. All statements are valid for three months from the date that they are signed.
The statement should be completed by a current employee of the employing organisation who either had oversight of your work or has access to your HR records.
The statement must:
- account for your entire time working for the organisation
- be completed by each of your previous employers if there was more than one individual, body or organisation during this period
- be completed by any individuals, bodies or organisations that you've provided healthcare services to, even if you were self-employed.
If you're self-employed, paid directly by patients and do not provide services to any third party, you may not need a statement. We'll let you know when we assess your application.
