Request an application form
Once you're ready to apply to re-enter the register of PAs and AAs, you'll need to email leavingandreturning@gmc-uk.org to request an application form. We can provide application forms in English or Welsh.
Sending documents to us
If you don't include copies of your documents when you submit your application, you can email them to leavingandreturning@gmc-uk.org.
Documents sent electronically by the issuer
If we receive a document by email directly from the issuer or signatory, we don't need to see the original. The document must be sent from a professional email address. We cannot accept evidence sent from publicly available email domains, such as Gmail, Yahoo or Hotmail.
