Registration and work history
You must provide details of all the medical regulatory authorities where you have held registration or a licence to practise in the last five years.
In addition, you must add all your work experience from:
- the last five years OR
- from the date your qualification was awarded, if this was less than five years ago.
Failure to provide any of this information could significantly delay your application or put your registration at risk.
Your fitness to practise declaration
We need to ask you for information regarding your fitness to practise before we can restore your licence.

The guidance will help you decide what you do and don’t need to tell us about. Failure to tell us about something you should have could significantly delay your application or put your registration at risk.
Please also note that in signing your declaration and submitting your application you are acknowledging that the GMC will complete global background checks on you.
