We investigate concerns about doctors so that we can protect the public and maintain professional standards. 

If we’ve asked you for a witness statement, it’s because we think you could assist our investigation into concerns about a doctor. 

Your statement will help us build a picture of what happened and decide whether we need to restrict or stop a doctor’s practice. 

We’ll often speak to a range of people to form a clear picture. It may be that you received care from the doctor, or you were working with the doctor when an incident took place. Even if you weren’t directly involved in events you might be able to help us with useful background information or documents.
