You’ll need to provide:

  • a fully completed application form
  • your name, address and contact details. Where applicable, also give your previous name if your current name does not match the name you qualified with
  • information about your qualification including name of awarding institution and dates of study leading to the award of your qualification

 

You'll also need to send copies of your:

  • qualification
  • passport
  • evidence confirming you have held (or still hold) registration in the country where your qualification was awarded OR another country where the profession is regulated by law
  • transcripts of course content
  • certified translations of any documents not issued in English.
