You’ll need to provide us with:

  • A fully completed application form.
  • Your name, address and contact details. Where applicable, previous name if your current name does not match the name you qualified with.
  • Information about your qualification including name of awarding institution and dates of study leading to the award of your qualification.
  • Copies of your:
    • Qualification
    • Passport
    • Evidence confirming you have held (or still hold) registration in the country where your qualification was awarded OR another country where the profession is regulated by law
    • Transcripts of course content
    • Certified translations of any documents not issued in English.
