1 He must notify the GMC within seven calendar days of the date these conditions become effective, or, if later, of the date that he obtains a medical post:
a of the details of his current post, including his job title, job location and responsible officer (or their nominated deputy) information;
b of the contact details of his employer and/or contracting body, including his direct line manager;
c of any training programmes he is in.

2 He must notify the GMC:
a of any post he accepts, before starting it;
b if any formal disciplinary proceedings against him are started by his employer and/or contracting body, within seven calendar days of being formally notified of such proceedings;
c if he applies for a post outside the UK.

3 He must allow the GMC to exchange information with any person involved in monitoring his compliance with his conditions.

4 a He must have a workplace reporter approved by his responsible officer (or their nominated deputy) and must inform the GMC of these arrangements.
b He must not start/restart work until his responsible officer (or their nominated deputy) has approved his workplace reporter and this approval has been forwarded to the GMC.

5 He must have an educational supervisor and must inform the GMC of
these arrangements.

6 a He must design a Personal Development Plan (‘PDP’), approved by his educational supervisor, to include the deficiencies in the following areas of his practice:
• Assessment of patients’ condition
• Providing/Arranging treatment
• Relationships with patients
• Record-keeping
• Basic Life Support
• Prescribing
• Chest pain
• Visual Field
b He must give the GMC a copy of his approved PDP within three months of these conditions becoming effective.
c He must give the GMC a copy of any subsequent approved PDP on request.
d He must meet with his educational supervisor as required, to discuss his achievements against the aims of his PDP.

7 a He must get the approval of his GMC Adviser before accepting any post.
b He must keep his professional commitments under review and limit his work if his GMC Adviser tells him to.
c He must stop work immediately if his GMC Adviser tells him to, and must get the approval of his GMC Adviser before returning to work.

8 He must not prescribe any drugs.

9 He must confine his medical practice to NHS training posts at a level no higher than ST1.

10 a He must be directly supervised, as defined in the ‘Glossary for undertakings and conditions’, in all of his posts by a clinical supervisor. His clinical supervisor must be approved by his responsible officer (or their nominated deputy) and he must inform the GMC of these arrangements.
b He must not start/restart work until his responsible officer (or their nominated deputy) has approved his clinical supervisor and this approval has been forwarded to the GMC.

11 He must not work:
a as a locum;
b out-of-hours;
c on-call;
d in any General Practice posts.

12 He must inform the following persons of the conditions listed at 1 to 11 above:
a his employer and/or contracting body;
b his responsible officer (or their nominated deputy);
c his immediate line manager at his place of work, at least 24 hours before starting work (for current and new posts);
d any prospective employer and/or contracting body, at the time of application.
