Work details
You need to tell us about details of any patient facing roles that you have undertaken in healthcare settings within the last five years.
Patient-facing roles in healthcare involve direct interaction and engagement with patients, providing care, support, or services that directly impact their well-being. You do not need to tell us about other work that is not a patient facing healthcare role, or administration healthcare support role or a laboratory role where you are not directly involved in the care of patients.
You also do not need to provide details of any work that you undertook that formed part of your course of study to become a PA/AA, such as placements or clinical attachments that were overseen by your course provider.
Employer reference forms
If you have worked in a patient-facing healthcare role in the last five years, you’ll need to complete an employer reference form that covers your most recent three months of employment. If you have/had more than one healthcare employer during this three-month period, we will need references from each employer.

As part of the application, you’ll be able to upload the completed form. If for any reason you cannot get a completed form, you can tick a box to tell us you can’t provide this. We’ll ask you to provide further information about this once we receive and assess your application for registration.
