On an unknown date between October and December 2023, at the request of a senior colleague (Dr A) Dr Ghouri forged at least nine patient feedback forms to form part of the supporting evidence for Dr A’s 2022-2023 appraisal and first GMC revalidation cycle. Dr Ghouri knew that the information contained within those forms was false, in that she had completed them without any patient input. Dr Ghouri’s actions were dishonest. This behaviour does not meet with the standards required of a doctor. It risks bringing the profession into disrepute and it must not be repeated. The required standards are set out in Good medical practice and associated guidance. In this case, paragraphs 81 and 89 of Good medical practice are particularly relevant: You must make sure that your conduct justifies your patients' trust in you and the public's trust in the profession. You must make sure any information you communicate as a medical professional is accurate, not false or misleading. This means: a. you must take reasonable steps to check the information is accurate Whilst this failing in itself is not so serious as to require any restriction on Dr Ghouri’s registration, it is necessary in response to issue this formal warning.
