1. To personally ensure the GMC is notified of the following information within seven calendar days of signing these undertakings: a the details of my current post, including i my job title ii my job location iii my responsible officer (or their nominated deputy) b the contact details for my employer and any contracting body, including my direct line manager c any organisation where I have practising privileges and/or admitting rights d any training programmes I am in e of the organisation on whose medical performers list I am included f of the contact details of any locum agency or out-of-hours service I am registered with. 2. To personally ensure the GMC is notified: a of any post I accept, before starting it b that all relevant people have been notified of my undertakings in accordance with undertaking [insert sequence number of U5] c if any formal disciplinary proceedings against me are started by my employer and/or contracting body, within seven calendar days of being formally notified of such proceedings d if any of my posts, practising privileges or admitting rights have been suspended or terminated by my employer or contracting body before the agreed date, within seven calendar days of being notified of the termination e if I apply for a post outside the UK. 3. To allow the GMC to exchange information with any person involved in monitoring my compliance with my undertakings. 4. a To have a workplace reporter appointed by my responsible officer (or their nominated deputy). b Not to work until: i my responsible officer (or their nominated deputy) has appointed a workplace reporter ii I have personally ensured that the GMC has been notified of the name and contact details of my workplace reporter. 5. a To get the approval of the GMC Adviser before accepting any post. b To keep my professional commitments under review and limit my work if the GMC Adviser tells me to. c To stop work immediately if the GMC Adviser tells me to and to get the approval of the GMC Adviser before returning to work. 6. To only work as a CT1/ST1 or clinical fellow (subject to approval by the GMC Adviser). 7. To get the approval of the GMC before starting work in a non-NHS post or setting. 8. Not to work hours more than 60% of whole time equivalent. 9. a To get the approval of my responsible officer (or their nominated deputy) and the GMC Adviser, before working [select the appropriate option]: i as a locum/in a fixed term contract ii out-of-hours iii on-call. b Not to work until: i My responsible officer (or their nominated deputy) and the GMC adviser has confirmed approval ii I have personally ensured that the GMC has been notified of the approval of my responsible officer (or their nominated deputy) and the GMC adviser. 10. Not to work in any locum post or fixed term contract of less than 3 months' duration. 11. a To be supervised in all my posts by a clinical supervisor, as defined in the Glossary for undertakings and conditions. My clinical supervisor must be appointed by my responsible officer (or their nominated deputy). b Not to work until: i my responsible officer (or their nominated deputy) has appointed my clinical supervisor and approved my supervision arrangements. ii I have personally ensured that the GMC has been notified of the name and contact details of my clinical supervisor and my approved supervision arrangements. 12. To personally ensure the following persons are notified of the undertakings listed at 1 to 12: a my responsible officer (or their nominated deputy) b the responsible officer of the following organisations: i my place(s) of work, and any prospective place of work (at the time of application) ii all my contracting bodies and any prospective contracting body (prior to entering a contract) iii any organisation where I have, or have applied for, practising privileges and/or admitting rights (at the time of application) iv any locum agency or out-of-hours service I am registered with v If any organisation listed at (i – iv) does not have a responsible officer, to notify the person with responsibility for overall clinical governance within that organisation. To work only in a Foundation Programme year two (F2) training post, / locum appointment for training (LAT) post approved by my postgraduate deanery, local education and training board (LETB) or foundation school, and my medical school. 6. a To get the approval of the GMC Adviser before accepting any post. b To keep my professional commitments under review and limit my work if the GMC Adviser tells me to. c To stop work immediately if the GMC Adviser tells me to and to get the approval of the GMC Adviser before returning to work. 7.a To design a personal development plan (PDP), approved by my responsible officer (or their nominated deputy) with specific aims to support my training. b To give the GMC a copy of my approved PDP within three months of the date I agree these undertakings. c To give the GMC a copy of my approved PDP on request. d To meet with my responsible officer (or their nominated deputy), as required, to discuss my achievements against the aims of my PDP. 8. a To have an educational supervisor approved by my responsible officer (or their nominated deputy) and to inform the GMC of these arrangements. b Not to start/restart work until my responsible officer (or their nominated deputy) has approved my educational supervisor and this approval has been forwarded to the GMC. 9. To get the approval of my responsible officer (or their nominated deputy) and the GMC Adviser, and to inform the GMC of the approved arrangements before working: a as a locum/in a fixed term contract b out-of-hours 10. a To be supervised in all my posts by a clinical supervisor, as defined in the Glossary for undertakings and conditions. 1. To notify the GMC promptly of any post he accepts for which registration with the GMC is required and provide the GMC with the contact details of his employer. 2. At any time that he is providing medical services, which require him to be registered with the GMC, to agree to the appointment of a workplace reporter nominated by his employer, or contracting body, and approved by the GMC. 3. To allow the GMC to exchange information with his employer, or any contracting body for which he provides medical services. 4. To inform the GMC of any formal disciplinary proceedings taken against him, from the date of giving these undertakings. 5. To inform the GMC if he applies for medical employment outside the UK. 6. To work with a Postgraduate Dean, (or their nominated deputy), to formulate a Personal Development Plan to support his training. 7. To forward a copy of his Personal Development Plan to the GMC within three months of the date on which he gives these undertakings. 8. To meet with his Postgraduate Dean, (or their nominated deputy), on a regular basis to discuss his progress towards achieving the aims set out in his Personal Development Plan. The frequency of the meetings is to be set by the Postgraduate Dean, (or their nominated deputy). 9. To allow the GMC to exchange information about the standard of his professional performance and his progress towards achieving the aims set out in his Personal Development Plan with the Postgraduate Dean, (or their nominated deputy), and any other person involved in his retraining and supervision. 10. At any time that he is employed, or providing medical services, which require him to be registered with the GMC, to place himself and remain under the supervision of an educational supervisor, as agreed by the GMC. His employer and/or Postgraduate Dean (or their nominated deputy) will be asked to assist in identifying a possible supervisor. 11. To obtain the approval of the GMC before accepting any post for which registration with the GMC is required. 12. To confine his medical practice to National Health Service training posts in Foundation Year 1 and Foundation Year 2, where his work will be supervised by a named Consultant. 13. To seek the advice of his Postgraduate Dean (or their nominated deputy), in relation to his future medical career. 14. To obtain the approval of his medical supervisor before accepting any post for which registration with the GMC is required. 15. Not to undertake any locum posts of less than 3 months duration. 16. To keep his professional commitments under review and limit his medical practice in accordance with his medical supervisor’s advice. 17. To cease work immediately if his medical supervisor advises him to do so. 18. To inform the following parties that his registration is subject to the undertakings listed at (1) to (17), above: a. Any organisation or person employing or contracting with him to undertake medical work b. Any locum agency or out-of-hours service he is registered with or applies to be registered with (at the time of application) c. In the case of locum appointments, his immediate line manager at his place of work (at least 24 hours before starting work) d. Any prospective employer or contracting body (at the time of application). 19. To permit the GMC to disclose the above undertakings to any person requesting information about his registration status. |
