To allow the GMC to exchange information with any person involved in monitoring my compliance with my undertakings. 4. a To have a workplace reporter appointed by my responsible officer (or their nominated deputy). b Not to work until: i my responsible officer (or their nominated deputy) has appointed a workplace reporter ii I have personally ensured that the GMC has been notified of the name and contact details of my workplace reporter. 5. a To design a personal development plan (PDP), with specific aims to address the deficiencies in the following areas of my practice. • Operative/Technical Skills b To get my PDP approved by my responsible officer (or their nominated deputy). c To give the GMC a copy of my approved PDP within three months of the date I agree these undertakings. d To give the GMC a copy of my approved PDP on request. e To meet with my responsible officer (or their nominated deputy), as required, to discuss my achievements against the aims of my PDP. 6. a To have an educational supervisor appointed by my responsible officer (or their nominated deputy). b Not to work until: i my responsible officer (or their nominated deputy) has appointed my educational supervisor ii I have personally ensured that the GMC has been notified of the name and contact details of my educational supervisor. 7. With the exception of Medico-Legal work, to get the approval of the GMC before starting work in a non-NHS post or setting. 8. Not to work: a. As a locum b. Undertake any out of hours work or on-call duties relating to knee replacement surgery. 9. a Except in life-threatening emergencies, not to carry out hip arthroplasty surgery. b To inform the GMC within seven calendar days of any occasion I carry out hip arthroplasty surgery in a life threatening emergency. 10. a Except in life-threatening emergencies, not to carry out knee arthroplasty surgery unless supervised by a clinical supervisor. b My clinical supervisor must be appointed by my responsible officer (or their nominated deputy). c Except in life-threatening emergencies, not to carry out knee arthroplasty surgery until: i my responsible officer has appointed my clinical supervisor and approved my supervision arrangements ii I have personally ensured that the GMC has been notified of the name and contact details of my clinical supervisor and my approved supervision arrangements. d To maintain a log detailing every knee arthroplasty surgery, which must be signed by the clinical supervisor. e To give the GMC a copy of this log on request. 11. To personally ensure my performance assessment report 23 July 2014 is shared with: a my responsible officer (or their nominated deputy) b the responsible officer of the following organisations i my place(s) of work, and any prospective place of work (at the time of application) ii all my contracting bodies, and any prospective contracting body (prior to entering a contract) iii any organisation where I have, or have applied for, practising privileges and/or admitting rights (at the time of application) v If any organisation listed at (i-iv) does not have a responsible officer, to notify the person with responsibility for overall clinical governance within that organisation. 1. To notify the GMC promptly of any post he accepts for which registration with the GMC is required and provide the GMC with the contact details of his employer. 2. At any time that he is providing medical services, which require him to be registered with the GMC, to agree to the appointment of a workplace reporter nominated by his employer, or contracting body, and approved by the GMC. 3. To allow the GMC to exchange information with his employer, or any contracting body for which he provides medical services. 4. To inform the GMC of any formal disciplinary proceedings taken against him, from the date of giving these undertakings. 5. To inform the GMC if he applies for medical employment outside the UK. 6. To work with his responsible officer (or their nominated deputy), to formulate a Personal Development Plan, specifically designed to address the deficiencies in the following areas of his practice in hip and knee surgery: a. Operative/Technical Skills 7. To forward a copy of his Personal Development Plan to the GMC within three months of the date on which he gives these undertakings 8. To meet with his responsible officer (or their nominated deputy), on a regular basis to discuss his progress towards achieving the aims set out in his Personal Development Plan. The frequency of the meetings is to be set by his responsible officer (or their nominated deputy). 9. To allow the GMC to exchange information about the standard of his professional performance and his progress towards achieving the aims set out in his Personal Development Plan with his responsible officer, (or their nominated deputy), and any other person involved in his retraining and supervision. 10. At any time that he is employed, or providing medical services, which require him to be registered with the GMC, to place himself and remain under the supervision of an educational supervisor, as agreed by the GMC. His responsible officer (or their nominated deputy) will be asked to assist in identifying a possible supervisor. 11. To disclose the Assessors’ Report of the assessment of his professional performance to: a. Any current and prospective employers, (at the time of application), b. Any other individual involved in his retraining and supervision. 12. To agree to undertake an assessment of his performance, at a date specified by the GMC, unless notified by the GMC that this assessment is not necessary. 13. a. Not to carry out knee or hip arthoplasty surgery unless directly supervised by a Consultant Orthopaedic Surgeon. b. To maintain a log detailing every case where he has undertaken knee or hip arthoplasty surgery, which must be signed by the supervising consultant. c. To provide a copy of this log to the GMC on a 3 monthly basis or, alternatively, confirm that there have been no such cases during that period. 14. To confine his medical practice to posts within the National Health Service and not undertake any private practice other than Medico-Legal. 15. Not to work as a locum. Not to undertake any out-of-hours work or on-call duties relating to hip and knee replacement surgery. 16. To obtain the approval of the GMC before accepting any post for which registration with the GMC is required. 17. To inform the following parties that his registration is subject to the undertakings listed at (1) to (16), above: a. Any organisation or person employing or contracting with him to undertake medical work b. His National Director of National Public Health Service (Wales) 18.
