Except in life threatening emergencies, not to carry out any operative procedure apart from the exceptions listed below:
• Fibro-optic endoscopy of the upper aerodigestive tract for diagnostic purposes.
• Examination of the ear using an operative microscope.
• The performance of aural toilet.
• Nasal cautery.
• Biopsies, subject to the conditions specified below, of:
o the ear for diagnostic purposes
o lesions of the upper aerodigestive tract for diagnostic purposes
o the skin of the head and the neck for diagnostic purposes
• Such biopsies are only permitted under local anaesthetic in outpatient settings.

3. To allow the GMC to disclose the above undertakings to any person requesting information about my registration status.

Undertakings

1) To personally ensure the GMC is notified of the following information within seven calendar days of signing these undertakings:
a the details of my current post, including
i my job title
ii my job location
iii my responsible officer (or their nominated deputy)
b the contact details for my employer and any contracting body, including my direct line manager
c any organisation where I have practising privileges and/or admitting rights
d any training programmes I am in
e of the contact details of any locum agency or out-of-hours service I am registered with.

2) To personally ensure the GMC is notified:
a of any post I accept, before starting it
b that all relevant people have been notified of my undertakings in accordance with undertaking 13
c if any formal disciplinary proceedings against me are started by my employer and/or contracting body, within seven calendar days of being formally notified of such proceedings
d if any of my posts, practising privileges or admitting rights have been suspended or terminated by my employer or contracting body before the agreed date, within seven calendar days of being notified of the termination
e if I apply for a post outside the UK.

3) To allow the GMC to exchange information with any person involved in monitoring my compliance with my undertakings.

4) a To have a workplace reporter appointed by my responsible officer (or their nominated deputy).
b Not to work until:
i my responsible officer (or their nominated deputy) has appointed a workplace reporter
ii I have personally ensured that the GMC has been notified of the name and contact details of my workplace reporter.

5) a To design a personal development plan (PDP), with specific aims to address the deficiencies in the following areas of my practice: -
• Maintaining Professional Performance
• Assessment
• Clinical Management
• Operative/Technical Skills
• Record Keeping
• Relationships with Patients
b To get my PDP approved by my responsible officer (or their nominated deputy).
c To give the GMC a copy of my approved PDP within three months of the date I agree these undertakings.
d To give the GMC a copy of my approved PDP on request.
e To meet with my responsible officer (or their nominated deputy), as required, to discuss my achievements against the aims of my PDP.

6) a To have an educational supervisor appointed by my responsible officer (or their nominated deputy).
b Not to work until:
i my responsible officer (or their nominated deputy) has appointed my educational supervisor
ii I have personally ensured that the GMC has been notified of the name and contact details of my educational supervisor.

7) To get the approval of the GMC before working in a non-NHS post or setting.

8) Not to work in any locum post or fixed term contract of less than 1-month duration.

9) a Except in life-threatening emergencies, not to carry out any operative procedure apart from the exceptions listed below. You may carry out the procedures listed below under the supervision of a clinical supervisor:
• fibre optic endoscopy of the upper aerodigestive tract for diagnostic purposes
• examination of the ear using an operating microscope
• the performance of aural toilet.
• Biopsies, subject to the conditions specified below, of
o the ear for diagnostic purposes;
o lesions of the upper aerodigestive tract for diagnostic purposes;
o the skin of the head and neck for diagnostic purposes
• Such biopsies are only permitted in an outpatients setting under local anaesthetic
b My clinical supervisor must be approved by my responsible officer (or their nominated deputy).
c Except in life-threatening emergencies, not to carry out any operative procedure except those specified in the list above. I must not undertake the procedures listed above until:
i my responsible officer has appointed my clinical supervisor and approved my supervision arrangements
ii I have personally ensured that the GMC has been notified of the name and contact details of my clinical supervisor and my approved supervision arrangements.
• If I wish to carry out any surgical procedures not listed above I must have the approval of my supervisor and be directly supervised.
d To maintain a log detailing every operative procedure except those specified in the list above, which must be signed by the clinical supervisor.
e To give the GMC a copy of this log on request.

10) a To be supervised in all my posts by a clinical supervisor, as defined in the Glossary for undertakings and conditions. My clinical supervisor must be appointed by my responsible officer (or their nominated deputy).
b Not to work until:
i my responsible officer (or their nominated deputy) has appointed my clinical supervisor and approved my supervision arrangements.
ii I have personally ensured that the GMC has been notified of the name and contact details of my clinical supervisor and my approved supervision arrangements.

11) To personally ensure my performance assessment report dated
15 January 2019 is shared with:
a my responsible officer (or their nominated deputy)
b the responsible officer of the following organisations
i my place(s) of work, and any prospective place of work (at the time of application)
ii all my contracting bodies, and any prospective contracting body (prior to entering a contract)
iii any organisation where I have, or have applied for, practising privileges and/or admitting rights (at the time of application)
iv any locum agency or out-of-hours service I am registered with
v If any organisation listed at (i-iv) does not have a responsible officer, to notify the person with responsibility for overall clinical governance within that organisation. If I am unable to identify the correct person, to contact the GMC for advice before working for that organisation
c the approval lead of my regional Section 12 approval tribunal (if applicable) - or Scottish equivalent
d my immediate line manager and senior clinician (where there is one) at my place of work, at least 24 hours before starting work (for current and new posts, including locum posts)
g my workplace reporter, educational supervisor and clinical supervisor.

12) To undertake an assessment of my performance, if requested by the GMC.

13) To personally ensure the following persons are notified of the undertakings listed at 1 to 14:
a my responsible officer (or their nominated deputy)
b the responsible officer of the following organisations:
i my place(s) of work, and any prospective place of work (at the time of application)
