1 She must personally ensure that the GMC is notified of the following information within seven calendar days of the date these conditions become effective:
a of the details of her current post, including:
i her job title
ii her job location
iii her responsible officer (or their nominated deputy)
b the contact details for her employer and any contracting body, including her direct line manager
c of any organisation where she has practising privileges and/or admitting rights
d of any training programmes she is in
e of the contact details of any locum agency or out-of-hours service she is registered with.

2 She must personally ensure the GMC is notified:
a of any post she accepts, before starting it
b that all relevant people have been notified of her conditions, in accordance with condition 5
c if any formal disciplinary proceedings against her are started by her employer and/or contracting body, within seven calendar days of being formally notified of such proceedings
d if any of her posts, practising privileges or admitting rights have been suspended or terminated by her employer before the agreed date within seven calendar days of being notified of the termination
e if she applies for a post outside the UK.

3 She must allow the GMC to exchange information with her employer and/or any contracting body for which she provides medical services.

4 a She must ask for a report from the person advising the GMC, for consideration by this tribunal, before any review hearing.
b She must keep her professional commitments under review and limit her work if recommended to do so by the person advising the GMC.
c She must stop work immediately if the person advising the GMC recommends that she does.

5 She must personally ensure that the following persons are notified of the conditions listed at 1 to 4:
a her responsible officer (or their nominated deputy)
b the responsible officer of the following organisations
i her place(s) of work and any prospective place of work (at the time of application)
ii all her contracting bodies and any prospective contracting body (prior to entering a contract)
iii any organisation where she has, or has applied for, practising privileges and/or admitting rights (at the time of application)
iv any locum agency or out-of-hours service she is registered with
v if any organisation listed at (i to iv) does not have a responsible officer, she must notify the person with responsibility for overall clinical governance within the organisation. If she is unable to identify this person, she must contact the GMC for advice before working for that organisation.
c the approval lead of her regional Section 12 approval tribunal (if applicable) - or Scottish equivalent
d her immediate line manager and senior clinician (where there is one) at her place of work, at least 24 hours before starting work (for current and new posts, including locum posts).

1. She must notify the GMC promptly of any post she accepts for which registration with the GMC is required and provide the GMC with the contact details of her employer and the NHS England Area Team on whose Medical Performers List she is included, or the local Health Board/Health & Social Care Board if employed in Scotland, Wales or Northern Ireland.

2. At any time that she is providing medical services, which require her to be registered with the GMC, she must agree to the appointment of a workplace reporter nominated by her employer, or contracting body, and approved by the GMC.

3. She must allow the GMC to exchange information with her employer or any contracting body for which she provides medical services.

4. She must inform the GMC of any formal disciplinary proceedings taken against her, from the date of this determination.

5. She must inform the GMC if she applies for medical employment outside the UK.

6. She must obtain the approval of the GMC before accepting any post for which registration with the GMC is required.

7. She must confine her medical practice to posts within the National Health Service and not undertake any private practice.

8. She must confine her medical practice to National Health Service training posts at a level no higher than ST1, where her work will be supervised by a named Consultant, and where an ST3/Registrar/Middle grade doctor/or above is available on site whilst she is on duty.

9. She must inform the following parties that her registration is subject to the conditions, listed at (1) to (8), above:
a. Any organisation or person employing or contracting with her to undertake medical work
b. Any locum agency or out-of-hours service she is registered with or applies to be registered with (at the time of application)
c. In the case of locum appointments, her immediate line manager at her place of work (at least 24 hours before starting work)
d. Any prospective employer or contracting body (at the time of application)
e. The NHS England Area Team in whose Medical Performers List she is included, or seeking inclusion (at the time of application), or the local Health Board/Health & Social Care Board if employed in Scotland, Wales or Northern Ireland.
f. Her responsible officer (or their nominated deputy)
g. Her regional Section 12 Approval Panel or in Scotland, any local Health Board or the State Hospitals Board with whom she is registered as an Approved Medical Practitioner under Section 22 of the Mental Health (Care and Treatment) (Scotland) Act 2003.
