5. a To design a personal development plan (PDP), with specific aims to address the deficiencies in the following areas of my practice. • Maintaining Professional Performance • Assessment of Patients’ Condition • Clinical Management • Record Keeping • Safety and Quality • Relationships with Patients b To get my PDP approved by my responsible officer (or their nominated deputy). c To give the GMC a copy of my approved PDP within three months of the date I agree these undertakings. d To give the GMC a copy of my approved PDP on request. e To meet with my responsible officer (or their nominated deputy), as required, to discuss my achievements against the aims of my PDP.

If I am unable to identify the correct person, to contact the GMC for advice before working for that organisation c the approval lead of my regional Section 12 approval tribunal (if applicable) - or Scottish equivalent d my immediate line manager and senior clinician (where there is one) at my place of work, at least 24 hours before starting work (for current and new posts, including locum posts) e my workplace reporter and educational supervisor [and clinical supervisor].
