Undertakings
on the registrant's registration

1. To notify the GMC within seven calendar days of signing these undertakings:
a of the details of his current post, including his job title, job location and responsible officer (or their nominated deputy) information
b of the contact details for his employer and/or contracting body, including his direct line manager
c of any organisation where I have practising privileges and/or admitting rights
d of any training programmes I am in
e of the contact details of any locum agency I am registered with

2. To notify the GMC:
a of any post he accepts, before starting it
b if any formal disciplinary proceedings against him are started by his employer and/or contracting body, within seven calendar days of being formally notified of such proceedings
c if I apply for a post outside the UK.

3. To allow the GMC to exchange information with any person involved in monitoring his compliance with his undertakings.

4 a To have a workplace reporter approved by his responsible officer (or their nominated deputy) and to inform the GMC of these arrangements.
b Not to start/restart work until his responsible officer (or their nominated deputy) has approved his workplace reporter and this approval has been forwarded to the GMC.

5 a To design a personal development plan (PDP), approved by his responsible officer (or their nominated deputy), with specific aims to address the deficiencies in the following areas of his practice.
• Patient assessment, examination and management
• Record keeping
• Multi-agency working
• Keeping up to date with relevant guidance, protocols, regulations and the law
b To give the GMC a copy of his approved PDP within three months of the date he agrees these undertakings.
c To give the GMC a copy of his approved PDP on request.
d To meet with his responsible officer (or their nominated deputy), as required, to discuss my achievements against the aims of his PDP.

6 a To have an educational supervisor approved by his responsible officer (or their nominated deputy) and to inform the GMC of these arrangements.
b Not to start/restart work until his responsible officer (or their nominated deputy) has approved his educational supervisor and this approval has been forwarded to the GMC.

7. To undertake an assessment of his performance, on a date given by the GMC, unless notified by the GMC that this assessment is not necessary.

8 a To get the approval of the GMC Adviser before accepting any post.
b To keep his professional commitments under review and limit his work if the GMC Adviser tells him to.
c To stop work immediately if the GMC Adviser tells him to and to get the approval of the GMC Adviser before returning to work.

9. To get the approval of the GMC before starting work in a non-NHS post or setting.

10. To only work in posts as a Specialty Doctor/ Staff Grade in Community Paediatrics.

11 a To be supervised in all his posts by a clinical supervisor, as defined in the Glossary for undertakings and conditions. His clinical supervisor must be approved by his responsible officer (or their nominated deputy) and he must inform the GMC of these arrangements.
b Not to start/restart work until his responsible officer (or their nominated deputy) has approved his clinical supervisor and this approval has been forwarded to the GMC

12. Not to work in any locum post or fixed term contract of less than 3 months duration.

13 To inform the following persons of the undertakings listed at (1) to (12):
a his employer and/or contracting body
b his responsible officer (or their nominated deputy)
c his immediate line manager at his place of work, at least 24 hours before starting work (for current and new posts, including locum posts)
d any prospective employer and/or contracting body, at the time of application
e the responsible officer of any organisation where he has, or has applied for, practising privileges and/or admitting rights, at the time of application
f any locum agency or out-of-hours service he is registered with

14. To allow the GMC to disclose the above undertakings to any person requesting information about his registration status.
