on the registrant's registration

1. You must personally ensure the GMC is notified of the following information within seven calendar days of the date these conditions become effective:
a the details of your current post, including:
i your job title
ii your job location
iii your responsible officer (or their nominated deputy)
b the contact details of your employer and any contracting body, including your direct line manager
c any organisation where you have practising privileges and/or admitting rights
d any training programmes you are in
e of the contact details of any locum agency or out of hours service you are registered with.

2. You must personally ensure the GMC is notified:
a of any post you accept, before starting it
b that all relevant people have been notified of your conditions, in accordance with condition 11
c if any formal disciplinary proceedings against you are started by your employer and/or contracting body, within seven calendar days of being formally notified of such proceedings
d any of your posts, practising privileges or admitting rights have been suspended or terminated by your employer before the agreed date within seven calendar days of being notified of the termination
e if you apply for a post outside the UK.

3. You must allow the GMC to exchange information with any person involved in monitoring your compliance with your conditions.

4. a. You must have a workplace reporter appointed by your responsible office (or their nominated deputy).
b. You must not work until:
i. Your responsible officer (or their nominated deputy) has appointed your workplace reporter.
ii. you have personally ensured tht the GMC has been notified of the name and contact details of your workplace reporter.

5. a You must get the approval of your GMC Adviser before accepting any post.
b You must keep your professional commitments under review and limit your work if your GMC Adviser tells you to.
c You must stop work immediately if your GMC Adviser tells you to and must get the approval of your GMC Adviser before returning to work.

You must get the approval of the GMC before starting work in a non-NHS post or setting.

7. You must not prescribe any drugs for yourself, or anyone with whom you have a close personal relationship.

8. a You must be supervised in all of your posts by a clinical supervisor, as defined in the Glossary for undertakings and conditions. Your clinical supervisor must be appointed by your responsible officer (or their nominated deputy).
b You must not work until:
i your responsible officer (or their nominated deputy) has appointed your clinical supervisor and approved your supervision arrangements
ii You have personally ensured that the GMC has been notified of the name and contact details of your clinical supervisor and your supervision arrangements.

9. a You must get the approval of your responsible officer (or their nominated deputy) and the GMC Adviser, before working as:
i a locum / in a fixed term contract
ii out-of-hours
iii on-call.
b You must not work until:
i your responsible officer (or their nominated deputy) and the GMC Advise has confirmed approval
ii you have personally ensured that the GMC has been notified of the approval of your responsible officer (or their nominated deputy) and the GMC Adviser.

10. You must not work in any locum post or fixed term contract of less than three months duration.

11. You must personally ensure the following persons are notified of the conditions listed at 1 to 10:
a your responsible officer (or their nominated deputy)
b the responsible officer of the following organisations:
i your place(s) of work, and any prospective place of work (at the time of application)
ii all your contracting bodies and any prospective contracting body (prior to entering a contract)
iii any organisation where you have, or have applied for, practising privileges and/or admitting rights (at the time of application)
iv any locum agency or out of hours service you are registered with.
v If any of the organisations listed at (i to iv) does not have a responsible officer, you must notify the person with responsibility for overall clinical governance within that organisation. If you are unable to identify this person, you must contact the GMC for advice before working for that organisation.
c your immediate line manager and senior clinician (where there is one) at your place of work, at least 24 hours before starting work (for current and new posts, including locum posts).
