on the registrant's registration

1. To notify the GMC within seven calendar days of signing these undertakings:
a of the details of her current post, including her job title, job location and responsible officer (or their nominated deputy) information
b of the contact details for her employer and/or contracting body, including her direct line manager
c of any organisation where she has practising privileges and/or admitting rights
d of any training programmes she is in
e of the organisation on whose medical performers list she is included
f of the contact details of any locum agency she is registered with.

2. To notify the GMC:
a of any post she accepts, before starting it
b if any formal disciplinary proceedings against her are started by her employer and/or contracting body, within seven calendar days of being formally notified of such proceedings
c if she applies for a post outside the UK.

3. To allow the GMC to exchange information with any person involved in monitoring her compliance with her undertakings.

4. a To have a workplace reporter approved by her responsible officer (or their nominated deputy) and to inform the GMC of these arrangements.
b Not to start/restart work until her responsible officer (or their nominated deputy) has approved her workplace reporter and this approval has been forwarded to the GMC.

5. a To get the approval of the GMC Adviser before accepting any post.
b To keep her professional commitments under review and limit her work if the GMC Adviser tells her to.
c To stop work immediately if the GMC Adviser tells her to and to get the approval of the GMC Adviser before returning to work.

6. To get the approval of the GMC before starting work in a non-NHS post or setting.

7. To get the approval of her responsible officer (or their nominated deputy) and the GMC Adviser, and to inform the GMC of the approved arrangements before working:
a as a locum/in a fixed term contract
b out-of-hours
c on-call.

8. Not to work in any locum post or fixed term contract of less than one month duration.

9. a To be supervised in all her posts by a clinical supervisor, as defined in the Glossary for undertakings and conditions. Her clinical supervisor must be approved by her responsible officer (or their nominated deputy) and she must inform the GMC of these arrangements.
b Not to start/restart work until her responsible officer (or their nominated deputy) has approved her clinical supervisor and this approval has been forwarded to the GMC

10.
a To only prescribe, administer, and have primary responsibility for drugs under arrangements that have been agreed with her responsible officer (or their nominated deputy) and the GMC Adviser, and to inform the GMC of these arrangements.
b Not to start/restart work until these arrangements have been agreed and the GMC has been informed of these arrangements.

11. Not to prescribe any drugs for herself, or a member of her family.

12. To inform the following persons of the undertakings listed at (a) to (g):
a her employer and/or contracting body
b her responsible officer (or their nominated deputy)
c her immediate line manager at her place of work, at least 24 hours before starting work (for current and new posts, including locum posts)
d any prospective employer and/or contracting body, at the time of application
e the responsible officer of any organisation where she has, or has applied for, practising privileges and/or admitting rights, at the time of application
f any locum agency or out-of-hours service she is registered with
g the organisation on whose medical performers list she is included or seeking to be included, at the time of application.

13. To allow the GMC to disclose the above undertakings to any person requesting information about her registration status.
