1. He will allow the GMC to exchange information with his employer, or any contracting body for which he provides medical services.

2. He will inform the GMC of any formal disciplinary proceedings taken against him, from the date of giving these undertakings.

3. He will work with the Postgraduate Dean of whichever Deanery he works under the jurisdiction of (the Postgraduate Dean), or such deputy as the Dean might nominate, to formulate a Personal Professional Development Plan (PDP) specifically designed to address his deficiencies in the following areas of his practice:
a. providing or arranging treatment
b. record keeping;
c. working within laws and regulations, in particular obtaining written informed consent.
d. providing or arranging investigations.

4. He will fulfil and complete the said PDP to the satisfaction of the Postgraduate Dean or his/her nominated Deputy.

5. He will forward a copy of his PDP to the GMC within 3 months of the date of commencing employment under the jurisdiction of the said Deanery.

6. He will meet or otherwise communicate with the Postgraduate Dean or his/her nominated deputy on a regular basis to discuss his progress towards achieving the aims set out in his PDP. The nature and frequency of the meetings/communication are to be determined by the Postgraduate Dean or his/her nominated deputy.

7. He agrees that the GMC may exchange information about the standard of his professional performance and his progress towards achieving the aims set out in his PDP to the Postgraduate Dean or his/her nominated deputy and any other persons involved in the implementation of his PDP.

8. He will agree to supervision when providing and arranging treatment in difficult and complex clinical situations by a registered medical practitioner of consultant or equivalent grade. He will ensure that a registered medical practitioner of consultant or equivalent grade will be available on call to him, but who may be off site, to provide that supervision. This undertaking will come to an end when the said supervisor confirms that he has no concerns about his ability in these situations.

9. He agrees not to perform laparoscopic colorectal surgery below the pelvic brim except under the auspices of a ‘buddy’ arrangement. The ‘buddy’ will be a trainer recognised as such by the Association of Laparoscopic Surgeons of Great Britain and Ireland (‘ALSGBI’) or of comparable standing and approved by the Postgraduate Dean. This undertaking will come to an end when the said trainer confirms that he is competent to perform the said surgery unsupervised and notifies the GMC of this.

10. He will undertake and complete any course in laparoscopic colorectal surgery directed by the said ‘buddy’.

11. He will, until the Postgraduate Dean decides that it is unnecessary, type, dictate and have typed or otherwise electronically produced all medical notes and records except those forms which cannot practicably be completed except in handwriting. He will use his best endeavours to complete such forms legibly.

12. He will undertake and complete within 6 months of these undertaking a course designed to improve his handwriting and will furnish to the GMC evidence of completion.
