Undertakings
on the registrant's registration

1. To notify the GMC promptly of any post he accepts for which registration with the GMC is required and provide the GMC with the contact details of his employer and the NHS England Area Team on whose Medical Performers List he is included, or the local Health Board/Health & Social Care Board if employed in Scotland, Wales or Northern Ireland.

2. At any time that he is providing medical services, which require him to be registered with the GMC, to agree to the appointment of a workplace reporter nominated by his employer, or contracting body, and approved by the GMC.

3. To allow the GMC to exchange information with his employer, or any contracting body for which he provides medical services.

4. To inform the GMC of any formal disciplinary proceedings taken against him, from the date of giving these undertakings.

5. To inform the GMC if he applies for medical employment outside the UK.

6. To obtain the approval of his medical supervisor before accepting any post for which registration with the GMC is required.

7. To keep his professional commitments under review and limit his medical practice in accordance with his medical supervisor’s advice

8. To cease work immediately if his medical supervisor advises him to do so.

9. To only prescribe drugs under arrangements which have been agreed between the GMC and the medical practitioner of consultant grade or equivalent supervising his work.

10. To obtain the approval of the GMC before accepting any post for which registration with the GMC is required.

11. To confine his medical practice to posts within the National Health Service and not undertake any private practice.

12. To confine his medical practice to general practice posts group practice of at least 3 members (including himself)

13. To confine his medical practice to general practice posts as a salaried GP, where his work will be supervised by a named GP

14. Not to work as a locum or undertake any out-of-hours work or on-call duties.

15. a. To maintain a log of all his prescriptions for controlled drugs.
b. To provide a copy of this log to the GMC on a 3 monthly basis or, alternatively, confirm that there have been no such cases.

16. To undertake and pass a Royal College of General Practitioners accredited Safeguarding Children Level 1 and 2 course, before returning to work as a General Practitioner.

17. To inform the following parties that his registration is subject to the undertakings listed at (1) to (16), above:
a. Any organisation or person employing or contracting with him to undertake medical work
b. Any locum agency or out-of-hours service he is registered with or applies to be registered with (at the time of application)
c. Any prospective employer or contracting body (at the time of application)
d. The NHS England Area Team in whose Medical Performers List he is included, or seeking inclusion (at the time of application), or the local Health Board/Health & Social Care Board if employed in Scotland, Wales or Northern Ireland.

18. To permit the GMC to disclose the above undertakings to any person requesting information about his registration status.
