Warnings are issued to indicate when a registrant's behaviour or performance is significantly below the standards expected, but when restricting a registrant's practice is not necessary. See more on warnings.

On multiple occasions between November 2008 and September 2024 Dr Kolpanowicz accessed their own medical records, issued prescriptions to themselves for various medications, ordered various tests for themselves and/or reviewed the results of their own tests.
When they did so, they failed to make it clear within the records which they made: that the consultations had not taken place with an independent medical practitioner; that they had self-prescribed, or ordered tests for themselves, or reported on their own test results; or the reason it was necessary for them to self-prescribe.
This conduct does not meet with the standards required of a doctor. It risks undermining public protection, specifically public confidence in the profession/professional standards and conduct and it must not be repeated. The required standards are set out in Good medical practice and associated guidance. In this case, paragraph 97 of Good medical practice is particularly relevant:
97. You must, wherever possible, avoid providing medical care to yourself or anyone with whom you have a close personal relationship. You must follow our more detailed guidance on Good practice in proposing, prescribing, providing and managing medicines and devices
Whilst this failing in itself is not so serious as to require any restriction on Dr Kolpanowicz’s registration, it is necessary in response to issue this formal warning.
