2. At any time that he is providing medical services, which require him to be registered with the GMC, to agree to the appointment of a workplace reporter nominated by his employer, or contracting body, and approved by the GMC.

4. To inform the GMC of any formal disciplinary proceedings taken against him, from the date of giving these undertakings.

7. To keep his professional commitments under review and limit his medical practice in accordance with his medical supervisor’s advice.

10. To refrain from all clinical work involving direct patient contact, unless prior agreement has been obtained from both his Medical Supervisor and the GMC.

b. Patients who have been referred from their GP for psychiatric assessment.

12. To limit the number of sessions he conducts in a week in line with the recommendations of his Medical Supervisor.

13. a. Except in life threatening emergencies, not to undertake consultations without a chaperone and a person trained in control and retraint techniques present. The chaperone must be a fully registered medical practitioner or fully registered nurse or medwife.

b. To maintain a log detailing every case where he has undertaken a consultation with such a patient, which must be signed by the chaperone.

c. To maintain a log detailing every case where he has undertaken a consultation with such a patient in a life-threatening emergency, without a chaperone present.

d. To provide a copy of these logs to the GMC on request or, alternatively, confirm that there have been no such cases during that period.

14 Not to work as a locum or undertake any out-of-hours work or on-call duties.

15. Not to undertake any physical examinations of patients or to undertake the management of a patient's physical health conditions.

16. To inform the following parties that her registration is subject to the undertakings listed at (1 ) to (15 ), above:

a. Any organisation or person employing or contracting with him to undertake medical work

b. Any prospective employer or contracting body (at the time of application)

c. His regional Section 12 Approval Panel.

17. To permit the GMC to disclose the above undertakings to any person requesting information about his registration status.
