Once you've collated your evidence, please email us on portfolio@gmc-uk.org to request an application form. Please list the following in your email:

  • your GMC number
  • the name of the sub-specialty you’re applying in
  • confirmation that you’ve completed approved training in the sub-specialty
  • attach a scanned copy of evidence of how you’re eligible to apply, e.g. a certificate or letter confirming you’ve completed approved training in the sub-specialty.

Once we've confirmed that you’re eligible to apply, we'll send you an application form. 

We won’t be able to process your application until we receive your fee.
