1 He must personally ensure that the GMC is notified of the following information within seven calendar days of the date these conditions become effective: a of the details of his current post, including: i his job title ii his job location. b the contact details for his employer and any contracting body, including his direct line manager c of the contact details of any locum agency or out-of-hours service he is registered with.

2 He must personally ensure the GMC is notified: a of any post he accepts before starting it b that all relevant people have been notified of his conditions, in accordance with condition 7 c if any formal disciplinary proceedings against him are started by his employer and/or contracting body, within seven calendar days of being formally notified of such proceedings d if any of his posts have been suspended or terminated by his employer before the agreed date within seven calendar days of being notified of the termination e if he applies for a post outside the UK.

4 He must get the approval of the GMC before starting work in a non-NHS post or setting.

5 a He must be supervised in all of his posts by a named clinical supervisor, as defined in the Glossary for conditions. His named clinical supervisor will: i. ensure any activity that involves patient contact, such as in person or remote consultations, examinations and procedures, is supervised at all times and in person ii. be present for home visits iii. be present with the PA for remote consultations iv. meet formally, fortnightly, for a case-based discussion and a feedback session. b He must not work until: i his senior clinician within the organisation has approved his supervision arrangements ii he has personally ensured that the GMC has been notified of these arrangements c He must provide a report from his named clinical supervisor in advance of or at his next interim measures review.

6 a Except in life-threatening emergencies, he must not carry out consultations with female patients without a chaperone present. b He must keep a log detailing every case where he has carried out a consultation with such a patient, which must be signed by the chaperone. c He must keep a log detailing every case where he has carried out a consultation with such a patient in a life-threatening emergency, without a chaperone present. d He must give a copy of this log to the case examiner or Interim Measures Tribunal (IMT) at his next interim measures review.

7 He must personally ensure that his immediate line manager and senior clinician at his place of work are notified of the conditions listed at 1 to 6 at least 24 hours before starting work (for current and new posts, including locum posts).
