4. I graduated in Australia / New Zealand / Hong Kong / Singapore / South Africa / West Indies and I hold/have held provisional registration with the GMC since before 19 October 2007
   

5. I graduated from the University of Malaya on or before 31 December 1989 and I hold/have held provisional registration with the GMC since before 19 October 2007
