This means you don't have a disease that you could pass onto someone else that could result in serious illness. 

No one has raised a concern about your health

This means your medical school or employer has never started a formal process because they were concerned about:

  • how you managed a health condition
  • how a health condition affected your ability to study or work as a doctor

No action has been taken against you by the police

This means you have never in the UK or overseas:

  • been arrested or interviewed about a violent or sexual offence
  • received a caution
  • received a conviction
  • received a penalty notice for disorder
  • had an anti-social behaviour or community order made against you
  • had other action taken against you (such as a fine, deferred prosecution agreement, contractual disclosure, facility agreement, restorative justice sanctions or similar).
  • No action has been taken against you by an organisation or person

    This means you have never in the UK or overseas:

  • been refused registration or a licence
  • been investigated by an organisation because of concerns about your fitness to practise
  • had a medical malpractice or negligence claim made against you.
  • I need to check whether to tell you about something
