1. Make an application on GMC Online or go back to your application if you are applying now.
2. Answer yes to the question on the page called “Your declaration of fitness to practise”.
3. In the text box below the question, tell us:

• what the condition is or was and how it affected you and your medical practice
• what process you went through, who with, and what the outcome was
• dates when your health condition affected your medical practice
• details of any treatment you received (including any engagement with specialists or support services)
• the status of the condition now (eg resolved, being managed, treatment is ongoing).
