How to use this guide for your application
You can use this page to help you answer the questions on your application. The questions cover health conditions and fitness to practise issues. This page explains how to apply this guide to different types of applications, including:
- joining the register for the first time or restoring your registration
- giving up your registration (voluntary erasure)
- restoring your licence to practise
- moving from provisional to full registration
- completing your annual return for revalidation (for doctors without a connection to a designated body).
The answers you give on your application are valid for three months. If your application takes more than three months to complete, we can ask you to answer the questions again before we approve it.
Being honest in your application
Honesty and integrity are at the heart of medical professionalism. What we expect of you is covered in Domain 4 - Trust and professionalism of Good medical practice.
You need to be honest when you apply. If you aren’t, we can refuse your application. If we find out you didn’t tell us about something relevant, we will investigate it to find out why and we might open a fitness to practise case.
You don’t need to tell us about all your health conditions and there are other things you don’t need to tell us. Read the rest of the guide to find out what you do and don’t need to tell us.

Joining the register or restoring your registration
When you join the register for the first time or restore your registration, we will ask you two questions about your health and seven questions about your fitness to practise. Each question we ask you has a page below designed to help you answer it. You don’t need to read all the pages only the ones most relevant to you:
Changing your registration status or submitting your annual return
If you are already registered, we will ask you one question about your fitness to practise when you apply to:
- give up your registration (voluntary erasure)
- restore your licence to practise
- complete the annual return for revalidation
- move from provisional to full registration.

If you have already told us about something, and your circumstances haven’t changed since your last application, you don’t need to tell us again.
