Reporting issues relating to consent and induction

The NHSLA and the GMC are committed to working together where our standards overlap. In addition to supervision of doctors in training, areas of overlap include induction and consent procedures, which are also being risk assessed by the NHSLA.

If concerns about improper consent-taking have been identified trusts may submit information on the steps they have taken to prevent recurrence using the form below. This information will be shared with the NHSLA for the risk assessment.
