Evidence you will need to submit
Once you have submitted your application to be released from the requirement to work in an approved practice setting, we will send you an email asking you to send us evidence that you meet the requirements for this application.
Your FACD if you have completed the Foundation Year 2 training programme
OR
If you have not completed the Foundation Year 2 training programme 12 months' satisfactory reports from your supervising consultant. These must cover your most recent 12 months' experience within the last three years and be in posts of at least three months duration.

You should pass the report forms to your supervising consultants. Once they have been completed by your supervising consultants they should send their report directly to the GMC.
If you have completed the Foundation Year 2 programme you must submit your FACD; we can not accept consultant reports.
