The employer’s statement
If you have not worked in a medical capacity for the past five years, or if you are self‑employed in private practice, please indicate this by
- Leaving the employer’s details blank and clicking the appropriate button if you are applying online
OR
- Ticking the appropriate box on page 3 of the form, and leave page 7 blank.
In all other cases please ensure that an employer’s statement is completed by your current or most recent employer(s), including if you are currently not working in a medical capacity but have done so at some point during the past five years or if you have been working in a medical capacity overseas.
Please note that the employer’s statement must account for your last three months of employment (this means your last three months worked, whether or not that work was spread out over a longer period, and not the last three calendar months). If you had more than one employer within those three months, please ask each of them to complete a separate copy of the employer’s statement.
