The Panel determined to impose an order of conditions on Dr Heinzman’s registration for a period of 18 months. The conditions are as follows:
- She must notify the GMC promptly of any professional appointment she accepts for which registration with the GMC is required and provide the contact details of her employer.
- She must allow the GMC to exchange information with her employer or any organisation for which she provides medical services.
- She must inform the GMC of any formal disciplinary proceedings taken against her, from the date of this determination.

She must confine her medical practice to posts within the National Health Service where her work will be supervised by a named Consultant(s) or General Practitioner(s).
- She must seek a report from her supervisor(s) for consideration by this Panel, prior to any review hearing of this Panel.
- She must only prescribe drugs under arrangements which have been agreed between the GMC and the consultant/General Practitioner supervising her work.
- She must not prescribe for herself, or a member of her family.
- She must not work as a locum or undertake any out-of-hours work or on-call duties.
- She must seek a report from the persons advising the GMC in relation to her fitness to practise, for consideration by this Panel, prior to any review hearing of this Panel.
- She must keep her professional commitments under review and limit her medical practice in accordance with the recommendations of the person(s) advising the GMC in relation to her fitness to practise.
- She must cease work immediately if any of the person(s) advising the GMC in relation to her fitness to practise recommend that she does so.
- She must inform the following parties that her registration is subject to the conditions, listed at (1) to (10), above:
- Any organisation or person employing or contracting with her to undertake medical work.
- Any prospective employer (at the time of application).
- The PCT in whose Medical Performers List she is included, or seeking inclusion (at the time of application).

The Panel reviewed the order of conditions imposed on Dr Kashyap’s registration on 21 March 2007 and determined that the conditions should be varied (currently due to expire on 20 September 2008).
The Panel further notified the Registrar of the GMC that an application should be made to the relevant Court for an extension of the interim order under section 41A (6) of the Medical Act 1983, as amended.
The varied conditions are as follows:
- He must notify the GMC promptly of any professional appointment he accepts for which registration with the GMC is required and provide the contact details of his employer.
- He must allow the GMC to exchange information with his employer or any organisation for which he provides medical services.
- He must inform the GMC of any formal disciplinary proceedings taken against him, from the date of this determination.
- He must inform the GMC if he applies for employment outside the UK.
- He must confine his medical practice to posts in orthopaedic surgery where his work will be supervised by a named Consultant Orthopaedic Surgeon (his workplace supervisor).
- He must seek a report from his supervisor, whether they be in the National Health Service or in a private hospital, for consideration by this Panel, which must be submitted, in writing, at least 14 days prior to any review hearing of this Panel.
- He must not carry out any unsupervised hip revision surgery on surgically fused hips.
- He must not carry out any complex surgical procedures, as defined by his Head of Department, unless directly supervised by a Consultant Orthopaedic Surgeon who will be present at all times in the operating suite.
- He must maintain a log detailing every case where he has undertaken a surgical procedure which must be signed by his workplace supervisor. Such a log should use the patient’s hospital number and initials to maintain anonymity and include details of the type of surgery undertaken.
- He must provide a copy of these logs to the GMC, at least 14 days prior to any review hearing of this Panel or, alternatively, confirm that there have been no such cases during that period.
- He must not undertake any locum posts of less than 3 months duration.
- He must only undertake on-call or out of hours duties at a named location, as approved by the GMC.
- He must comply with the ongoing investigation by the GMC into his fitness to practise and agree to undergo any assessment required.
- He must continue to work with his mentor, Dr Beaumont, or another mentor (named to and approved by the GMC). Evidence of compliance with this condition must be provided in writing at least 14 days prior to any review hearing.
- He must arrange for any new mentor to be provided with a copy of the GMC’s December 2006 Performance Assessment Report.
- His workplace supervisor must supervise his daily work. Such supervision should include:
- monitoring his operative workload
- ensuring, if appropriate, that he has sufficient time to give supervision and support to junior colleagues
- monitor and ensure that his routine performance is satisfactory
- ensure that all appropriate investigations are carried out upon his patients
- ensure he gives accurate information to patients and that he obtains full and informed consent
- ensure he accurately completes all required patient records and notes
- ensure that he undertakes and complies with any relevant departmental audits
- ensure that any delegated care is appropriate and that patients do not suffer as a result of such delegation.
- He must inform the following parties that his registration is subject to the conditions, listed at (1) to (16) above:
- Any organisation or person employing or contracting with him to undertake medical work.
- Any locum agency or out-of-hours service he is registered with or applies to be registered with (at the time of application).
- Any prospective employer (at the time of application).

The Panel further notified the Registrar of the GMC that an application should be made to the relevant Court for an extension of the interim order under section 41A (6) of the Medical Act 1983, as amended.
The varied conditions are as follows:
- He must notify the General Medical Council (GMC) promptly of any professional appointment he accepts for which registration with the GMC is required and provide the contact details of his employer.
- He must allow the GMC to exchange information with his employer or any organisation for which he provides medical services.
- He must inform the GMC of any formal disciplinary proceedings taken against him, from the date of this determination.
- He must inform the GMC if he applies for medical employment outside the UK.
- He must confine his medical practice to NHS posts in psychiatry where his work will be supervised by a named Consultant.
- He must seek a report from your supervisor(s) for consideration by this Panel, prior to any review hearing of this Panel.
- He shall not undertake locum posts of less than one week’s duration.
- He must not undertake any private practice.
- He must maintain a log of his Continuing Professional Development (CPD) activities, which may include evidence of courses attended, audits performed, presentations made and on line CPD activities and confirmation of completion of these. He must provide a copy of this log to the GMC at least 14 days prior to any review hearing of this Panel.
- He must arrange for his current consultant supervisor, or any future consultant supervisor to be provided with a copy of the GMC’s February 2008 Performance Assessment Report.
- He must inform the following parties that his registration is subject to the conditions listed at (1) to (10) above:
- Any organisation or person employing or contracting with him to undertake medical work.
