Please give us the full address details of the medical regulatory body you want us to send a CGS to. You can find the details of individual regulatory bodies in our CGS database

We have agreements with many international regulators to send CGSs by email. If we don't have an agreement with your regulator, we'll send the CGS by first class post or, for overseas regulators, by airmail.

If you need us to send a CGS to your overseas employer, please request this in writing to:

3. Gender
4. Date of birth
5. Nationality at the point of registration
6. Primary Medical Qualification
7. Current status on the Register
8. Date of provisional registration (if applicable)
9. Date of full registration
10. Date of current entry
11. Specialist Register entry (if applicable)
12. GP Register entry date (if applicable)
13. Address
